29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:     On or about August 23, 2014 and prior to that time employees were ladder accessing  roof warning line system area without an established point of access warning line system, utilizing a trash chute which was not protected by standard guardrails, disposing of waste materials directly from the roof edge without utilizing a trash chute with ground to eave distances in excess of 30 feet and the employer had not initiated and maintained established safety and health program policies which addresses  points of access connection points to warning line systems, guarding points of access and disposal of waste materials.

29 CFR 1926.501(a)(2): Employees were allowed to work on surfaces which did not have the requisite strength and structural integrity:     On or about August 23, 2014 and prior to that time employees performing low sloped associated roofing worked in areas adjacent to unprotected sides or edges along the perimeter exterior where the fiberglass felt paper over hung from 6" to 12" allowing employees to step or work where the walking/working surface did not have the requisite strength and structural integrity to support employees working on that surface exposing employees to fall potentials in excess of 30'.

On or about August 23, 2014 and prior to that time employees were performing low sloped associated roofing work on an approximate 98' x 98' roof deck with unprotected sides or edges without conventional fall protection  and along the east side of the structure utilizing a waste material disposal trash chute which was not protected by standard guardrails in that both sides of the trash chute guardrail system were not protected by midrail protection in that the guardrail system on each side had 24"w x 43"h openings in the system below the top rail exposing employees to fall potentials in excess of 30'.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge:    On or about August 23, 2014 and prior to that time employees were performing low sloped associated roofing work on an approximate 98' x 98' roof deck with unprotected sides or edges without conventional fall protection and with warning lines being erected  less than 6 feet from the unprotected side or edge in that along the south side adjacent to the ladder access area the warning line ranged from 2'5" to 4'6" from the edge, along the east side area adjacent to the trash chute the warning line ranged from Y connection points at the trash chute erected at the edge to 5" from the unprotected side or edge and areas along the west side warning lines ranged from 3' to 5'6" from the unprotected side or edge exposing employees to fall potentials in excess of 30'.
