On or about August 23, 2014 and prior to that time employees were performing low sloped associated roofing work on an approximate 98' x 98' roof deck with unprotected sides or edges without conventional fall protection and along the east side of the structure utilizing a waste material disposal trash chute which was not protected by standard guardrails in that both sides of the trash chute guardrail system were not protected by midrail protection in that the guardrail system on each side had 24"w x 43"h openings in the system below the top rail exposing employees to fall potentials in excess of 30'.
