29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)  NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE.  On or about 08/27/14 located at 4855 Willow Lake Ln Cumming, GA:  Employees installing asphalt shingles while working from an unprotected 4 / 12 pitch roof were exposed to fall hazards of approximately 22 feet.

On or about at 08/27/14 located at 4855 Willow Lake Ln, Cumming, GA:  Employees were observed using a pneumatic nail gun without safety glasses.
