29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:    a.      On or about August 22, 2014, at the Hernando Desoto Bridge, West Memphis, Arkansas, employees accessed the diaphragm using a Genie S-65 boom supported elevated work platform and steeping off of the platform onto the  diaphragm form  which extended beyond and below the bridge deck.  When standing on the wooden form, employees were not protected from falls to the ground below.     b.      On or about August 22, 2014, at the Hernando Desoto Bridge, West Memphis, Arkansas, employees working in the diaphragm near the north end were exposed to the hazards of falls from to the ground below.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:    On or about August 22, 2014 and prior thereto, at the Hernando Desoto Bridge, West Memphis, Arkansas, reinforcing steel bars protruded horizontally from the concrete bridge girders exposing employees walking on the bridge deck and climbing into the diaphragm to the hazard of falling onto the reinforcing steel bars and being impaled.
