29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press.    On or about August 20, 2014, employee(s) assigned to hand-feed parts into the point of operation of the 22 Ton, Johnson Machine & Press Corp., full revolution mechanical power press (press #2), were exposed to the unguarded pinch point created between the dies. Employee(s) were exposed to hazards that were likely to result in amputation injuries.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.332(b)(1): Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.    Employees were not adequately trained in and familiarized with required electrical safety related work practices that pertained to troubleshooting energized circuits.  Employees were exposed to contact with energized parts and/or arc flash/blast hazards.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.333(a): Safety-related work practices shall be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards.    Employees were assigned to perform voltage testing and troubleshooting on energized circuits operating at up to 480 volts AC without appropriate protection from electrical hazards, including arc flash/blast and/or contact with energized parts.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
