29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  (a) In the Utility Plant, the exit doors were locked and required a keycard to enter and exit. At the exit was a release button under a protective shield that releases the locked door requiring more than a single action to open the door.  Also, there was inconsistency from door to door and took additional time to look for and locate the release for each door.

29 CFR 1910.36(d)(2): Exit route doors were operated by a device or alarm that could restrict emergency use of the exit route if the device or alarm fails.  (a) In the Utility Plant, the exit doors were locked and require a keycard to enter and exit. At the exit was a release button under a protective shield that releases the locked door.  Should the release fail to operate properly, employees would be trapped.

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points:  (a) In the Utility plant, an exit access between 2 inch raised concrete platforms measured approximately 26 inches wide with piping hanging down on one side and a pole on the opposite side. The exit access did not meet the minimum width of 28 inches at all points.  IN THE ALTERNATIVE:  29 CFR 1910.37(a)(3): Exit routes were not free and/or unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp were not provided where the exit route was not substantially level:  (a) In the Utility plant, an exit route was not substantially level in that the route traveled in a groove measuring approximately 26 inches wide between 2-inch raised concrete platforms.  Additionally there was piping hanging down on one side and a pole on the opposite side. This condition created a trip hazard.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  (a) In the Utility plant boiler plant area, a sign was not posted near the exit access route indicating that direction of travel from the main aisle to the nearest exit north as the route traveled north then jogged west due to piping and other equipment.  This condition created an entrapment hazard.

29 CFR 1910.37(d)(3): Employees were exposed to hazards of flammable or explosive substances or equipment used during construction, repairs, or alterations, that are beyond the normal permissible conditions in the workplace, or that would impede exiting the workplace.  (a) In the Utility plant chiller area, on the northeast side, at the exit discharge in the pump room, construction and associated equipment impeded the exit route as there was a skid steer located outside within 5 feet of the door impeding the route to a safe location.

29 CFR 1910.304(b)(4)(ii)(B): Where connected to a branch circuit supplying two or more receptacles or outlets, a receptacle supplied a total cord- and plug-connected load in excess of the maximum specified in Table S-4:        (a) In the break room of the Utility plant, a Pizza Oven operated at 1235 watts and a Whirlpool Microwave MT2145SJB operated at 13.6 amps were powered from a duplex receptacle through a 15 amp Model SP6 Relocatable Power Tap (RPT) E128811 thereby exceeding the rated capacity of the RPT which overloaded the relocatable power tap and the receptacle.
