29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded.    The employer does not ensure that every floor hole into which employees can accidentally walk are guarded. This violation was observed on or about August 25, 2014, at the flooring above the mud pits area of the rig# 730 where employees were exposed to a trip and fall hazard while working around the holes in the steel grating without the holes covered or guarded.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that every floor hole into which employees can accidentally walk are guarded.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve.  The employer does not ensure that all respirators are stored so that they cannot become contaminated.  This violation was observed on or about August 25, 2014, in the doghouse area of the rig# 730 when two 3M half face tight fitting respirators were found stored in an area that did not protect them from damage, contamination and dust.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all respirators are maintained and cared for so that they cannot become contaminated or damaged.
