29 CFR 1926.304(d):  Each portable, power-driven circular saw withdrawn from  the work did not automatically and instantly return the lower guard to the  covering position of the saw.    (a)  Front of Building - On or about August 27, 2014 employees were exposed to laceration hazards while working or walking near a portable circular saw and the blade guard was jammed by a wood block in the unguarded position.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    (a)  Front of Building - On or about August 27, 2014 employees were exposed to fall hazards while working from a partially erected scaffold at a working level of 8 feet 11 inches above the floor and they were not protected by the working level being fully floored.

(a)  Front of building - On or about August 27, 2014 an employee was exposed to a 12.5 foot fall hazard while working and sitting on the top plate and he was not protected by use of guardrail systems, safety net systems, or personal fall arrest systems.    (b)  Middle of building - On or about august 27, 2014 an employee was exposed to an 8 to 9 foot fall hazard while standing on 1st floor joists and helping to install building front joists and he was not protected by use of guardrail systems, safety net systems, or personal fall arrest systems.
