29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  On or about August 20, 2014:  a) Employee was exposed to fall hazards of approximately 12 feet while installing siding on a residential structure, utilizing a ladder jack scaffold without the use of a personal fall arrest system.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about August 20, 2014:  a) Employee using a ladder jack scaffold at a height of approximately 12 feet was not wearing a personal fall arrest system. The employer failed to properly train any employee on a scaffold system to be able to minimize the hazard.
