29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Employees performing oil well maintenance including removal of the hood were exposed to fall hazards associated with an approximate height of 6 to 12 feet and the employer did not ensure appropriate fall protection methods were implemented.
