29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms where the entire platform was not attached to the fork.  The employer does not ensure that the entire platform is securely attached to the forks when forklifts are used to support scaffold platforms.  This violation was observed on or about August 26, 2014 on the east side of the structure, where employees were  performing stucco work from a height of 7 feet 4 inches, while using a platform without the platform attached to the forklift.     Pursuant to 29 C.F.R 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the entire platform is attached to the forks when using a forklift to support scaffold platforms.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  The employer does not provide fall protection for employees working 6 feet or more above a lower level.  This violation was observed on or about August 26, 2014 on the east side of the structure where employees were exposed to a fall of 7 feet 4 inches while performing stucco work from a scaffold platform without a fall protection system.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are provided fall protection when working at heights of 6 feet or more above a lower level.
