29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. The employer does not protect each employee working on a scaffold more than 10 feet above a lower level from falling. This violation was observed on or about August 27, 2014, at the job site where an employee was exposed to a fall hazard of 12 feet when working from the elevated forks of a fork lift without being protected from falling. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee working on a scaffold more than 10 feet above a lower level is protected from falling.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: The employer does not firmly secure a safety platform to the forks when using a fork lift to elevate personnel. This violation was observed on or about August 27, 2014, at the job site where an employee was exposed to a fall hazard of 12 feet when working from the elevated forks of a fork lift without the employer securing a safety platform to the forks. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each a safety platform is firmly secured to the forks when using a fork lift to lift personnel.
