29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about 4 September 2014, and at times prior thereto, a roofer was using a pneumatic nail gun to install roofing shingles without the use of eye protection, exposing the roofer to the hazard of being struck by flying objects to the face or eyes.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 4 September 2014, and at times prior thereto, three workers were performing roofing activities on a second story residence under construction without the use of fall protection, exposing the roofers to a fall hazard of approximately 11 feet 5 inches and more to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about 4 September 2014, and at times prior thereto, three roofers accessed the roof of a residence under construction using an extension ladder that was not extended at least 3 feet above the upper landing, exposing the roofers to a fall hazard of approximately 11 feet 5 inches to the ground below.
