29 CFR 1910.146(c)(4): If the employer decides that its employees will enter permit spaces, the employer shall develop and implement a written permit space program that complies with this section. The written program shall be available for inspection by employees and their authorized representatives:  a. On or about June 9, 2014, at the refuse facility, the employer did not develop and implement a written permit confined space program regarding drivers entering the body of the refuse trucks to clean or remove excess debris.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:   a.  On or about June 9, 2014, at the refueling station, the electrical Fill-Rite pumps, model FR701V electrical conduit was pulled away from the pump housing, exposing employees to a shock hazard.
