29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 4 September 2014, and at times prior thereto, a framer was installing black insulation boards at the first floor roof for the second story without having head protection, exposing the framer to the hazard of being struck by falling materials, tools, or debris.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about 4 September 2014, and at times prior thereto, two framers were using pneumatic staplers to perform framing activities without the use of eye protection, exposing the framers to the hazard of being struck by flying objects to the face and eyes.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about 4 September 2014, and at times prior thereto, a framer was installing black insulation boards while working from the beams of the top of the first floor roof near the edge of the roof without fall protection, exposing the framer to a fall hazard of approximately 9 feet 3 inches to the slab below, or approximately 22 feet 3 inches to the ground adjacent to the slab below.
