Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   (a) Jobsite, 640 Lamberd Avenue, Johnstown, PA 15904: On or before September 4, 2014, employees were working unprotected and exposed to fall hazards of between approximately 20 feet 4 inches to 10 feet 3 inches while preparing to apply shingles on a 3/12 sloped roof.
