29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) Site - On or about 09/02/14 and at times prior thereto workers  performed brick masonry work from the platform and the bracket of the tubular welded frame scaffold.  The platforms were not completely planked and employees were exposed to 16 ft. falls to ground level.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification.

A ladder was not installed.
