29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:    a) Excavation - On or about 09/02/2014, construction workers were exposed to a struck-by hazard from falling rock, soil, and unsupported adjacent structures while installing formwork in an excavation and while not having been provided head protection.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures:    a)  South Side of Excavation  On or about 09/02/2014, during their excavation of a 14 foot deep building foundation, the contractor undermined a concrete patio and did not provide a support system to ensure the patio did not collapse.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.651(k)(2): Where the competent person finds evidence of a situation that could result in a possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety:   a)  Excavation - On or about 09/02/2014, workers were not removed from a building excavation where the designated site competent person was aware that necessary protective systems were not in place, adjacent structures which had been undermined had not been supported and where spoils piles were located within 1 ft. of the unprotected edge of the open exaction and posed a struck-by hazard.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

All protruding reinforcing steel, onto and into which employees could fall, shall be guarded to eliminate the hazard of impalement:   a)  Excavation  On or about 09/02/2014, construction works installing reinforcing steel and concrete formwork were exposed to an impalement hazard from 18 inch vertical rebar dowels which were not covered or otherwise protected.
