29 CFR 1910.147(c)(5)(i):  Hardware was not provided by the employer for isolating or blocking of machines or equipment from energy sources:    In the yard, employees performed high pressure testing of pipes and valves. Employees were exposed to being struck by flying metal, under pressure. The employer did not have barriers which were high enough to protect nearby employees and the barriers were arranged such that there were spaces between the individual barriers which flying metal could pass between.

29 CFR 1910.107(c)(6): Electrical wiring, motors, and other equipment outside of but within twenty (20) feet of any spray area, and not separated therefrom by partitions, produced sparks under normal operating conditions and did not otherwise conform to the provisions of subpart S of this part for Class I, Division 2 Hazardous Locations:    In the shop area, an employee was painting valves in a spray area and there was a non-explosion proof electrical outlet, which was within twenty (20) feet of the spray area.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used:  In the spray paint area, an employee performed spray painting of valves and was exposed to KEM 400 Enamel, Silver metallic, by the Sherwin-Williams Company. The employer did not provide a respirator that the employee was used to fit test the employee.
