29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Along the Eastern Rake Edge - On or about September 4, 2014, an employee, setting the shingles on the 8:12 pitch roof, was not protected from a fall hazard of 23.5 feet by the use of a guardrail system, safety net system, or personal fall arrest system, or alternative fall protection. b) Along the North Edge - On or about September 4, 2014, two employees, installing roofing material on the 4:12 pitch roof, were not protected from a fall hazard of 20.3 feet by the use of a guardrail system, safety net system, personal fall arrest system, or alternative fall protection. c) On the Garage Roof - On or about September 4, 2014, an employee, setting the shingles on the 8:12 pitch roof, was not protected from a fall hazard of 7.5 feet by the use of a guardrail system, safety net system, or personal fall arrest system, or alternative fall protection. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At the Garage Roof - On or about September 4, 2014, the ladder used to access the roof did not extend 3 feet above the eave, exposing employees to a fall hazard of 7.5 feet. b) At the Southeastern Corner - On or about September 4, 2014, the ladder used to access and egress the residential roof did not extend 3 feet above the landing, exposing employees to a fall hazard of 12.7 feet. No abatement certification or documentation required.
