29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about September 5, 2014, employees were exposed to fall hazards of approximately 25 feet when they were working on steep roofs with no fall protection or improper fall protection.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  On or about September 5, 2014, employees were exposed to fall hazards of approximately 25 feet when they were working on steep roofs and had not properly adjusted their harnesses to fit.  Employees had returned to the roof during the inspection after donning harnesses that were on site.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about September 5, 2014, employees were exposed to fall hazards of approximately 25 feet when the ladder they were using to access the roof did not extended three feet above the edge of the roof.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.   On or about September 5, 2014, employees were exposed to fall hazards of approximately 25 feet when the employees had not been trained to recognized ladder hazards such as, but not limited to,   a)     Ladders that did not extend three feet above the edge of the roof  b)     Ladders that had broken rungs  c)     Ladders that did not have access to the bottom rungs
