29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. On or about September 5, 2014, employees were exposed to fall hazards of approximately 25 feet when they were working on steep roofs and had not properly adjusted their harnesses to fit. Employees had returned to the roof during the inspection after donning harnesses that were on site.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about September 5, 2014, employees were exposed to fall hazards of approximately 25 feet when the ladder they were using to access the roof did not extended three feet above the edge of the roof.
