29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a) Facility, 1569 Pittsburgh Road, Uniontown, PA:  On or about September 4, 2014, the work area below and around the Delta Radial arm saw, model # 651 had an accumulation of sawdust ranging from 12 to 18" high, 4' across and 2' deep, creating a fire hazard.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a) Facility, 1569 Pittsburgh Road, Uniontown, PA:  On or about September 4, 2014:  An employee was operating the Wheelabrator, serial # 3696, and was exposed to struck-by hazards due to the metal, upright fixed stop guard at the end of the roller, being bypassed by a piece of wood manually placed underneath, that holds the metal stop paddle in the flat, open position, therefore not stopping a beam at the end of the roller.

29 CFR 1910.213(h)(1): Radial saw(s) did not have an upper hood constructed in such a manner and of material that would have protected the operator from flying splinters, broken saw teeth, etc. and that deflected sawdust away from the operator:    a) Facility, 1569 Pittsburgh Road, Uniontown, PA:  On or about September 4, 2014, employees were exposed to laceration hazards due to the guard on the Dewalt radial arm saw, model # 651, being stuck in the upright position and exposing the blade 3-6 inches.
