29 CFR 1910.1096(i)(2): All individuals working in or frequenting any portion of radiation area(s) were not instructed in the safety problems associated with exposure to radioactive materials or radiation and in precautions or devices to minimize exposure:    (a) On or about September 9, 2014, Wellocity Chiropractic & Wellness Center LLC did not provide employee(s) operating the Bennett IMAGER digital x-ray imager, with instruction on the type of radiation given off, health hazards of the radiation, or how to protect themselves from potential radiation exposure from the imager.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1096(i)(3): The employer did not post a current copy of 29 CFR 1910.1096, and a copy of the operating procedures applicable to the work conspicuously in such location(s), to ensure that employees working in or frequenting radiation areas would observe these documents on the way to or from their place of employment:  (a) On or about September 9, 2014, Wellocity Chiropractic & Wellness Center LLC did not post a current copy of 29 CFR 1910.1096, and a copy of the operating manual for the Barrett IMAGER digital x-ray imager in an area where employee(s) frequenting the radiation areas would observe and have access to these documents.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
