29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:    On or about September 8, 2014, at the Village of the Seven Mornings Apartments worksite at 1400 Harkrider in Conway, Arkansas, front of Building E, wooden boards with protruding nails were scattered on the ground.  This condition exposed employees to puncture hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about September 8, 2014, at the Village of the Seven Mornings Apartments worksite at 1400 Harkrider in Conway, Arkansas, Building E, employees on a pitched roof engaged in roof repair activities were exposed to unprotected falls of greater than 6 feet.
