(b) on the veneer scaffold located on the roof, the cantilevered platform that employees worked from to perform masonry tasks extended beyond 18 inches over its support. These conditions exposed employees to falls from elevations.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: On or about September 9, 2014, at the worksite located at 8201 Cisterna Way in Fort Smith, Arkansas, front south side of home construction, a safe access was not provided for employees to access supported scaffold work platforms. (a) Employees accessed and dismounted the 2nd level work platform by climbing the scaffold end frames. (b) Employee dismounted from the 2nd level work platform and climbed down the cross braces to lower to ground level. These conditions exposed employees to slips and falls from elevation.
