29 CFR 1903.19(d)(1): The employer did not submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required. a) The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 for hazards associated with 1-3) employees exposed to a fall hazard over 10 feet using a ladder jack scaffold to paint the eaves of a church without utilizing fall protection and employees not trained on the limitations on the height of the ladder jack scaffold and the recognition of the greater than 10 foot fall hazard and the procedures to follow to minimize exposure to said hazard.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system; On or about September 8, 2014 at 15 Warren St. Lebanon, a church building, an employee was exposed to a 32 ft. fall hazard while standing on a ladder jack scaffold painting the eave of the building without wearing a personal fall arrest system.
