The fall hazard to the employee was approximately 15 feet to the ground.

The employee was not trained to recognize the hazards of working at heights and how to protect against them.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) Jobsite, 422 Reiling Lane, Altoona, PA: On or about September 10, 2014: An employee was using an 8 foot Werner portable step ladder to access the roof. The ground to eave height outside the house was approximately 12 feet, which left about a 2 foot gap from the top of the ladder to the roof on the inside of the house, where the ladder was set up.
