29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Jobsite, 422 Reiling Lane, Altoona, PA:  On or about September 10, 2014, an employee was sheathing a residential roof and was not protected from falling.  The fall hazard to the employee was approximately 15 feet to the ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Jobsite, 422 Reiling Lane, Altoona, PA:  On or about September 10, 2014, An employee was sheathing a roof at approximately 15 above the ground was not protected from falling.  The employee was not trained to recognize the hazards of working at heights and how to protect against them.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    a) Jobsite, 422 Reiling Lane, Altoona, PA:  On or about September 10, 2014:  An employee was using an 8 foot Werner portable step ladder to access the roof.  The ground to eave height outside the house was approximately 12 feet, which left about a 2 foot gap from the top of the ladder to the roof on the inside of the house, where the ladder was set up.
