At the perch area:      (a) At Wilcox Road and Lakeshore Road in Tampa, for Excel Impact Crusher model 5700, serial number J0400 had missing guards located near the area where the cleaning person responsible for removing metal debris and garbage from the belt, on or about September 10, 2014.    b)  At Wilcox Road and Lakeshore Road in Tampa, for Excel Impact Crusher model 5700, serial number J0400 had missing guards observed over the moving chains did not cover the entire moving part area, on or about September 10, 2014.
