29 CFR 1910.147(c)(1): Energy control program. The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a)    CB&I Stone & Webster, Inc., did not establish an energy control program consisting of energy control procedures, employee training, sharing their program with the host company Haverhill Chemicals, Inc., and periodic inspections for their nested employees such as but not limited to maintenance and capital employees who perform servicing and maintenance of equipment and piping systems with energy sources such as but not limited to; electric, hydraulics, thermal heat, and chemicals, within the process and the piping systems at Haverhill Chemicals, Inc., located at 1019 Haverhill-Ohio Furnace Rd, Haverhill, OH, 45636 on a daily basis.

29 CFR 1910.147(d)(6): Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment have been accomplished:  (a)    On August 27, 2014, prior to starting work on the A1401 950 ton temporary chiller piping system, CB&I employees did not adequately verify that isolation of the knife valve had been accomplished in that the knife valve was leaking cold then hot water while employees were replacing the 12" x 10" reducing flanges exposing employees to burns.
