29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment are not to be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use.    On or about September 8, 2014, the employer allowed the use of a Skytrak 6036 rough terrain forklift to elevate employees in an unapproved personnel work platform.  The manufacturer does not allow personnel to be elevated by this rough terrain forklift exposing the employee to a fall hazard.

29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval.     On or about September 8, 2014, the employer added an unapproved personnel work platform to the forks of a Skytrak 6036 prior to getting approval from the manufacturer.  The personnel work platform did not meet all safety design requirements of the manufacturer and the ANSI B56.6, Safety Standard for Rough Terrain Forklift Trucks standard:   a.  The personnel work platform did not have at least one lanyard anchorage point installed for each occupant, exposing employees to a fall hazard.    b.  The personnel work platform floor dimensions exceeded the width of the forklift wheel base by more than 10 inches.
