29 CFR 1910.67(c)(2(v): Employee was not wearing a body belt with lanyard attached to the boom or basket when working from an aerial lift:     a)  Employee was not wearing a harness and tied off to the aerial lift when installing a sign with the lift truck located in the street. Employee can fall from the lift bucket if the if the bucket bounces due to lift malfunction or if struck by a vehicle.      Location: 250 Park Ave S., New York, N.Y. (at 20th Street)    On or about 9/15/2014    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
