29 CFR 1910.147(d)(3)  All energy isolating devices that are needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    For employee(s) who  performed deep cleaning of the mixer bowls on Mixers 3, 4 and 5 while the bowl was tilted by the 5-ton Yale under-slung crane, the mixer was locked out but the crane was not locked out.

29 CFR 1910.179(m)(1) A thorough inspection of all ropes was not made at least once a month and a certification record which included the date of inspection, the signature of the person who performed the inspection and an identifier for the ropes which were inspected was not kept on file and readily available to appointed personnel:  In the Mixing room, the running wire rope belonging to the 5-ton Yale under-slung crane was bent.  This wire rope was used to tilt the mixer bowls at Mixers #3, #4, and #5.  Monthly inspection records were not conducted and maintained.

29 CFR 1910.1200(h)(3)(i): Employee training did not include the methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.):    On or about 6/14/2014 in the Mixer department, a fire occurred in Mixer #8 which set off the sprinkler system.  Other fires have occurred in the Mixers in the past several years which required employees to use a fire extinguisher.  Mixer operators are exposed to a serious burn hazard.    OR IN THE ALTERNATIVE:    29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from these hazards, including specific procedures the employer has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used:    On or about 6/14/2014 in the Mixer department, a fire occurred in Mixer #8 which set off the sprinkler system.
