29 CFR 1926.501(b)(2)(i):  Each employee who is constructing a leading edge 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.      On or about September 8, 2014, the employer did not ensure employees installing the leading edge of a roof deck were in use of fall protection exposing the employees to a fall hazard of more than 6 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.     On or about September 8, 2014, the employer did not ensure the side rails of an extension ladder being used by employees to gain access to the upper level of a building extended at least 3 feet above the landing exposing employees to a fall hazard.
