29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   a) Job site, 1400 Main Street, Canonsburg, PA: On or about September 16, 2014, during construction activities, the employer did not ensure that employees used eye/face protection when operating a power saw to cut wood.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    a) Job site, 1400 Main Street, Canonsburg, PA: On or about October 7, 2014, during construction activities, the employer did not ensure that only electric cords that were in good repair were used in that an employee was cutting with a power saw and the cord had a hole cut through the jacket showing the inner wires, it had several sliced areas taken out the of jacket in other areas, and one area near the base of the saw appeared to have been melted.
