On or about December 4, 2014, the employer did not ensure that each floor hole on the walkway in front of the caustic tanks in the Die Prep/Die Cleaning area were protected by a cover that leaves no openings of more than 1 inch wide.

29 CFR 1910.23(d)(1): Flights of stairs having four or more risers were not equipped with a standard railing as specified in paragraphs (d)(1)(i) through (v) of this section:    On or about December 4, 2014, the employer did not ensure that the stairway in the caustic area was equipped with a standard railing.    a) As specified in (d)(1)(iii), on stairways less than 44 inches wide having both sides open, the stairway did not have a stair railing on both sides.    b) The existing railing did not meet the requirements of a standard railing in that it did not have an intermediate railing.      American Aluminum Extrusion, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(d)(1), which was contained in OSHA inspection number 991068, citation number 1, item number 4 and was affirmed as a final order on November 13, 2014, with respect to a workplace located at 1 St. Lawrence Avenue, Beloit, WI 53511.
