29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.    (a) Location - 276 Salem Street, Medford, MA:  On or about 8/21/2014, employees were installing roof shingles from a ladder jack scaffold platform that extended greater than 18 inches over its support.

(a) Location - 276 Salem Street, Medford, MA:  On or about 8/21/2014, employees were installing roof shingles from a ladder jack scaffold that exceeded twenty feet in height.     Shaun P. McDonough was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.452(k)(1), which was contained in OSHA inspection number 977223, citation number 01, item number 1b and was affirmed as a final order on 7/8/2014 with respect to a workplace located at 100 Main Street, Stoneham, Massachusetts.

(a) Location - 276 Salem Street, Medford, MA:  On or about 8/21/2014, an employee installing roof shingles on a roof that was greater than 6 feet above a lower level was not protected from falling to the ground below.     Shaun P. McDonough was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 977223, citation number 01, item number 2a and was affirmed as a final order on 7/8/2014 with respect to a workplace located at 100 Main Street, Stoneham, Massachusetts.
