Employees were exposed to crush by, struck by and fall hazards.

On or about September 19, 2014 and at times prior employees were allowed to operate a forklift without verification of operator training. On or about September 19, 2014 and at times prior to employees were exposed to crushed by, struck by, and fall hazards when a forklift used to elevate employees with an attached platform had hydraulic leaks and low tire pressure.  The powered industrial truck was not taken out of service until restored to a safe operating condition as required by ANSI B56.1-1969 paragraph 606 A.
