29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  (a) 16 South Park Street, Reading PA - Employee(s) were observed performing roofing work activity on a 8 in 12 pitch residential roof without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 23 feet to the ground below, on or about September 23, 2014.      No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     (a)  16 South Park Road Reading, PA - Employees were observed using a aluminum extension ladder that did not extend at least 3 feet above the landing surface at the garage, thereby exposing employees to a fall hazard of approximately 9 feet 1 inch to the ground below, on or about September 23, 2014.    No abatement certification or documentation required.
