29 CFR 1926.20(b)(2):  The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a) Jobsite - on or about September 24, 2014 - the employer did not provide regular inspections of the jobsite by a competent person.  Observed hazards include the lack of Personal Protective Equipment being used by employees, ladder set-up and use, fall protection on the steep pitched roof deck and the lack of first aid supplies on the site.

29 CFR 1926.95(a):  Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  (a) Building Roof - on or about September 24, 2014 - an eye injury hazard existed on site.  Employees that were using pneumatic nail guns to install roof shingles were not wearing any eye protection, such as safety glasses with side shields or goggles.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Building Roof - on or about September 24, 2014 - a fall hazard ranging between 10 feet, 8 inches to 19 feet existed on site.  Employees were installing new roof shingles on a 7 on 12 pitched residential roof deck without any conventional fall protection.
