29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work had the strength and structural integrity to support employees safely.  Employees were allowed to work on surfaces which did not have the requisite strength and structural integrity to support employees safely:    (a) Former Foundry Building, 4831 Front Mountain Road, Belleville, Pennsylvania - The employer did not determine that all parts of the roof where employee's were walking / working had the required strength and structural stability to safely support the employees.  On September 23, 2014, an employee fell through a section of the roof decking to the floor 32 feet below, suffering fatal injuries.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:    (a) Former Foundry Building, 4831 Front Mountain Road, Belleville, Pennsylvania - Employees worked to repair the roof deck and to install insulation panels on the western part of the building, 32 feet above the ground.  To access this flat roof, employees used a fixed ladder at the east end of the 24-foot high roof, walked across the 24-foot high flat roof, and then used another fixed ladder on the 24-foot high roof to get to the 32-foot high roof.  The employer had installed a warning-line around the work area on the 32-foot high roof but did not connect the points of access to the work area by installing warning lines up to the ladders and by using warning lines to form an access path across the 24-foot high roof between the two ladders, exposing employees to the hazard of falling to a lower level, on or about September 23, 2014.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder:    (a) Former Foundry Building, 4831 Front Mountain Road, Belleville, Pennsylvania - Employees worked to replace the roof on the western part of the building.  The employer provided and employees used a 24-foot long extension ladder for accessing the 24-foot high roof on the south side of the building.  Although the employer had the ladder secured at its top to a rigid support that did not deflect, the employer did not provide a grasping device to assist employees in mounting and dismounting the ladder, exposing employees to a fall to a lower level, on September 24, 2014.    Corrected during the inspection.  No abatement certification and documentation required.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date of the training, and the signature of the person who conducted the training or the signature of the employer:    (a) Former Foundry Building, 4831 Front Mountain Road, Belleville, Pennsylvania - Employees worked to repair the roof deck on the former foundry building, 32 feet above the ground below, which involved the need to use fall protection.  The employer did not prepare a written certification record to verify that the employer provided a training program for each employee who might have exposure to fall hazards so that employees could minimize exposure to these hazards, on September 23, 2014.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
