29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) On or about August 20, 2014, and at times prior, the employer Earnest T. Johnson, did not establish a program consisting of an energy control procedure, employee training, and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative. An employee was exposed to struck by hazards, in that maintenance/washing of the amusement rides was performed without isolating the energy source(s). Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) On or about August 22, 2014, and at times prior, the employer Earnest T. Johnson, did not ensure that live parts of electric equipment, operating at more than 50 Volts, were guarded against accidental contact by use of approved cabinets or enclosures. The bottom portion of the center drive unit on the Silver Streak ride, containing live parts of electrical equipment, was not guarded against accidental contact, exposing an employee to contact with electrical hazards while cleaning/washing the ride.
