29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 09/25/2014 on the rooftop of building 7922, where the roof (6/12 slope) was having shingles installed, the use of fall protection, i.e. harness, lanyard or lifeline was not used exposing employees to a 9 ft. fall hazard.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   a) On or about 09/25/2014 certification of training was not completed to verify employees had received training to recognize fall hazards and procedures to follow to minimize fall hazards. Employees were exposed to a 9 ft. fall hazard.
