On or about 09/26/14 at 101 Industrial Park Dr., Perry, GA: Employee was exposed to fall- hazard, while preforming inspections standing on top of the bus roof next to the AC unit. Employee was exposed to a 10 foot fall hazard. The employer failed to ensure that the bus was in the appropriate guardrail scaffold system.
