Workers were observed engaged in excavation/trenching operations installing 8.0 inch ductile sewer pipes to bypass an existing manhole. Worker(s) were exposed to potential cave-in and struck-by hazards from loose spoils that were on the located on the south side of the 16.0 foot deep excavation.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: Workers were engaged in excavation/trenching operations installing 8.0 inch, SDR 35, ductile sewer pipes to bypass an existing manhole separating a sanitary and storm water piping system. Worker(s) were exposed to potential cave-in and struck-by hazards and regular and frequent inspections by a competent person were not conducted to ensure that the jobsite, materials, and equipment such as cave-in protection systems and proper distance of spoil piles were maintained and utilized in a safe manner.
