29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On September 18, 2014 and at times prior thereto; East side/second floor area; an employee framing a residential home was observed working on the top plate without fall protection in place to prevent them from falling 20 feet to ground level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) On September 18, 2014 and at times prior thereto; First floor; the ladder used to access the work area (second floor) was 12 inches below the working platform exposing them to a fall hazard of 10 feet to the lower level while accessing or stepping off the ladder to the platform.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a) On September 18, 2014 and at times prior thereto; First floor; the step ladder used to access the work area (second floor) was 12 inches below the working platform having the employees to use the top step and exposing them to a fall hazard of 10 feet to the lower level while accessing or stepping off the ladder to the platform.
