OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:    a) Bailey's Mills Rd, Harding NJ    Employees worked on Mack rear-loading compacting garbage truck without receiving training on the hazards related to working on or around a garbage truck.    Violation occurred on or about 9/30/2014    b) Bailey's Mills Rd, Harding NJ    Employees operated a Mack rear-loading compacting garbage truck in speeds of 20-28miles/hr and with distances traveled exceeding 0.2 miles while employees were standing on the riding step.    Violation occurred on or about 9/30/2014    c) Bailey's Mills Rd, Harding NJ    Employees operated a Mack rear-loading compacting garbage truck while an employee was standing on the loading sill.    Violation occurred on or about 9/30/2014

The employer did not record workplace injuries, which occurred on 6/24/2014 and 9/30/2014, on the OSHA 300 log until documentation was requested.    Violation occurred on or about 10/16/2014.
