29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a.)    Jay Huddle Storage, Inc. located at N810 CR 11 Napoleon, OH 43545; On or about October 1, 2014, the employer did not ensure a written program consisting of energy control procedures, employee training, and periodic inspections was implemented. Employees were required to weld steel plates on the vegetable washer drum and change out blades on the carrot crowning equipment and were exposed to hazards from unexpected start up of the equipment.

Jay Huddle Storage, Inc. located at N810 CR 11 Napoleon, OH 43545; On or about October 1, 2014, the employer did not assure employees were protected from the point of operation while using saws in the Crowning Room. Employees are required to crown carrots and are exposed to amputation hazards from the unguarded saw blades.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  a.)    Jay Huddle Storage, Inc. located at N810 CR 11, Napoleon, OH 43545; On or about October 1, 2014, the employer did not assure employees were protected from the chain and sprocket on the vegetable washer in the Wash Bay. Employees empty the scrap bin and work around the vegetable washer and are exposed to caught by hazards.
