On or about October 1, 2014 - at the above addressed jobsite, the employee who was required to operate the CJB 300 Rough Terrain Forklift did not receive initial training, thereby exposing employees to struck by hazards.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. On or about October 1, 2014 - at the above addressed jobsite, an employee performing masonry operations from a tubular welded frame scaffold outrigger plank, approximately eighteen (18) feet three (3) inches above the ground, was exposed to injuries from falls due to lack of side rails.
