29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights.  On or about August 1, 2014 the employer did not ensure that a mobile scaffold was fully planked at the job site exposing employees to fall hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.  On or about October 1, 2014 the employer did not ensure that employees working on a scaffold in excess of ten feet were protected from falling at the job site exposing employees  to fall hazards.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner.  On or about October 1, 2014 the employer did not ensure that scaffold wheels were secured so as to prevent the scaffold from moving while employees were working above exposing employees to fall hazards at the job site.
