29 CFR 1910.95(d)(3): Repeat employee exposure monitoring was not performed whenever a change in production, process, equipment or controls increases noise exposure:  a) Pallet Building department - An employee manufacturing pallets was exposed to continuous noise at 134.9% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of approximately 92.2 dBA during the 315 minute sampling period on October 7, 2014. The exposure calculation included a zero increment for the 165 minutes not sampled. The employee had not received an annual audiogram.
