29 CFR 1910.334(a)(2)(ii):  There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service until repairs and tests necessary to render the equipment safe were made:    1) On October 15, 2014, Master Stone, LLC, as the creating employer, did not ensure that employees of JP Marble & Granite were protected from injuries while operating a granite saw that had strain relief pulled loose at the plug end of the cord.
